New Patient Appointments
Before treatment can begin, a psychiatric evaluation is required. This is a one time procedure* that begins with filling out all necessary forms and questionnaires in advance of your appointment. (takes 30-60 min). Then, at least one hour in Dr. Martin's office. (More details contained in the new patient paperwork packet by clicking the "Get Forms" button).
A $50.00 registration deposit is required to set up your first appointment. This charge will be deducted from the total fee of your first visit and is totally refundable if you cancel 24 or more hours in advance.
We accept Visa, MasterCard, American Express and Discover.

1. Use the patient's name to enroll in our Online Scheduler on the right side of this page. After completely filling out the form, we highly recommend that you write down your username and password. You will need that info every time you login. When you are finished, click on the "Enroll" button.
2. Now click the "Make Appointment for Psychiatric Evaluation" button on the right side and pick an appointment time.
IMPORTANT!
3. After setting your appointment, click on the "Get Forms" button below. Print the required new patient forms, fill them out completely and bring them to your appointment. Failure to bring completed paperwork may result in your appointment being rescheduled, (which is inconvenient for both of us).
